PARENT FORMS


REGULAR SCHOOL YEAR REGISTRATION
 The following documents are required to enroll your child:

  • Completed Registration Form and Parent Participation Agreement

  • Registration Fee ($100 for returning families, $120 for new families)

  • Emergency Card (please submit an updated form)

  • Immunization Record (bring original – we will copy for you)

  • Birth Certificate (bring original – we will copy for you)

  • Parent Handbook Acknowledgement

  • Best of Care Form

SUMMER REGISTRATION

  • Emergency Card (please submit an updated form)
